In this Department of Medicine Clinical Pathological Conference (CPC), Dr. Jung Seo presents the case of a 71-year-old man with bullous pemphigoid on dupilumab and a history AV heart block requiring a permanent pacemaker who presented for medical care with four months of fevers and new skin lesions. Dr. Mikel Etchegaray serves as Case Discussant; Dr. Diego Camacho serves as Radiology Discussant; and Dr. Auris Huen serves as Pathology Discussant.

Needs Statement

Faculty and trainees within the Department of Medicine who are involved in the care of patients need to be regularly updated with the recent advances and guidelines in the fields of internal medicine and quality improvement to increase clinical knowledge, and to implement the advances and guidelines in clinical practice to enhance the quality of patient care and outcomes.

Educational Objectives

At the conclusion of this activity, the participant should be able to:

  • Interpret clinical data in the context of a clinical question to tease out relevant information.
  • Recognize the role of PET scan in differentiating between infection and bland thrombus.
  • Consider infections with opportunistic pathogens in patients receiving immunosuppressants.
