The College Precision Medicine Quality Improvement Initiative
National Institute of Health Definition
Precision Medicine: The innovative approach that takes into account individual differences in patients’ genes, environments, and lifestyles to curate clinical treatment plans.
Goal
Apply the latest evidence-based science to improve clinical practice in high priority areas at an academic medical center and to make it simple to apply at the front line level.
Protocol
Develop a clinical protocol for targeted conditions and integrate select initiatives into the clinical workflow to:
- Best Practice Advisory (non-interruptive) Identify specific patients who may benefit
- Clinical decision support - Smart Set
- Point-of-care education - QRG, Care Algorithm, Patient Education (tracking for credit outside of encounter)
- Recognize the work and learning 30 ABIM Practice Assessment Points or completion of one ABFM Improvement Project that clinicians are doing to improve care
- QI Project Streamlined to Improve Care
Screening for Primary Aldosteronism in Treatment Resistant Hypertension
Recent evidence demonstrates that primary aldosteronism has a prevalence much higher than previously recognized, is associated with increased risk for cardiovascular and renal sequelae when compared to patients with essential hypertension at similar blood pressures, and current screening is limited.
Screening for Primary Hyperparathyroidism in Patients with Hypercalcemia
Recent evidence demonstrates it is the most common cause of outpatient hypercalcemia, underdiagnosis occurs across broad practice settings, and risk of fragility fractures, kidney stones, decline in kidney function, and neuropsychiatric symptoms.
