Dr. Mahveen Sohail is a geriatrician with special interest in clinical innovation, geriatric co-management, and interprofessional geriatric education. She is a staff physician at Baylor St. Luke's Medical Center with special emphasis on frailty, delirium prevention, and advanced care planning. She is focused on readmission risk reduction and has developed the Transitions of Care Clinic at the Baylor St. Luke's Senior Care Clinic where she sees vulnerable older adults who are deemed high risk for readmission. She also directs the Geriatric Medicine rotation for the Internal Medicine residency at Baylor College of Medicine. In addition to her clinical and educational duties, Dr. Sohail serves as one of the faculty leaders of the Southeast Texas Geriatric Workforce Enhancement Program (SETx-GWEP), a five-year initiative focused on geriatric interprofessional education in primary care.
